Tax forms

What forms do you offer?

Due to changes in federal tax laws, for Tax Year 2018, individuals will no longer use Form 1040A or Form 1040EZ, but instead will use the redesigned Form 1040. Many people will only need to file Form 1040 and no schedules.

The forms listed below are what the library has to offer.  The forms we have are located along the wall on your left as you enter the library.  These forms are free to you.

  • Federal Form 1040 + instruction booklet

How can I get forms you don't offer?

If the Federal or State tax form you need is not at the library, in most cases, you can print the form you need off the IRS website (federal) at www.irs.gov/forms-instructions or the Iowa Department of Revenue website (State) at https://tax.iowa.gov/individuals

You will need Adobe Acrobat to open and download any forms off the websites.  Also, please note that the IRS will not accept some forms printed from the web page, such as the W-2.  The directions will specify this.  As an alternative, you can use one of the library computers to print them yourself from either the IRS or the State website or ask one of the staff members to help you.  The cost to print is $.15 per page for a black & white copy.
